Part-Time Discharge Planner - Mental Health Professional

Position: Part-time Discharge Planner (LSW, LCSW, APSW, LPC, LMFT)

Facility: Olmsted County Jail

Location: Rochester, MN

Hours/Shift: 8 hours per week, flexible

  • Responsible for detainee’s mental health discharge plan.
  • Responsible for creating and maintaining relationships with community agencies that provide services to people with mental illness or serious medical conditions.
  • Facilitate applications for financial assistance eligibility.
  • Initiate process regarding resumption of benefits for detainees awaiting release.
  • Responsible for abiding in security regulations, including limits of confidentiality of detainee information requirements of the facility.
  • Assist medical and mental health care team in transfer of detainee to mental health hospital or emergency detention when necessary.  
  • Document services provided to the detainee in the health record, including contacts with community providers after detainee’s release from custody.
  • Maintain tracking logs of any follow-up contacts provided after detainee’s release from facility.
  • Attend Mental Health meetings and In-Service trainings as directed by clinical or outreach supervisor.
  • Perform other duties as assigned.


  • Degree in a behavioral/social science field (e.g., Psychology, Social Work) from an accredited college or university. 
  • Current license in good standing. - Licensed Clinical Social Worker LCSW, Advanced Practice Social Worker APSW, Licensed Professional Counselor LPC, Licensed Marriage and Family Therapist LMFT
  • Active CPR certification
  • Must be able to practice independently.
  • Coursework and professional experience that indicates knowledge of mental health counseling, group and individual psychotherapy, diagnosis and treatment of mental disorders, psychological assessment techniques, crisis intervention, and mental health consultation.
  • Must be able to apply principles of critical thinking to a variety of practical and emergency situations and accurately follow standardized procedures that may call for deviation.
  • Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.  
  • Must obtain and maintain security clearance.
